A 33 year-old andrew leipus is brought to theER by his mother with compliants of bizarre behaviour. He is wearing a wool coat, several sweaters, a wool cap, and mittens on the hottest day of the year. His amood changes from anger to laughter instantly. His speech and behavior are childlike and when asked what brought him to the hospital, he replies," his mom's car".His mother also reports she has heard him talking to himself in his bedroom. When she questioned him, he denied that he was speaking, but then continued to speak when she left the room. His mother stated that his problems have escalated since he stopped taking his medication several months ago. His first hospitalization occurred at the age of 18 and he has been hospitalized more than 15 times since then.which one of the following statements is true? a. this patient has BEST prognosis among schizophrenics b.the auditory hallucinations he had are indicative of the defence mechanism PROJECTION c.earlier onset of the illenss means usually good prognosis d.repeated admissions are indicative of more postive symptoms e.his answers to doctor's question is an example of abstract thinking.

It's disorganised schizophrenia, and have worse prognosis so answer a is wrong( FOM KAPLAN NOTES)/ and later onset has better prognosis so answer c is wrong/ answer e is wrong coz they have problem with abstract thinking/ so left with B and D, and sorry Vyseerx.. I really don't know but taking a guess here I will pick B, coz d seems wrong too, coz I think.repeated admission doesn't tell about positive symptoms..positive symptoms are DELUSION, HALLUCIANTION, AND STRAGE BEHAVIOUR. PLEASE KINDLY DO EXAPLIN THANKS

the auditiory hallucinations that the patient have are nothing but the defence PROJECTION. that is the clearcut answer. the unshakable truth of any MCQ exam is there is a clear cut answer to each and every question. if you can not figure out that, is usually because of ignorace of one particular principle. i hope you got it now.take care

I don't get it that auditory hallucination is the defense mechanism of projection. Projection by definition is attibuing one's own peronally unacceptable feelings to others. vyseerx, could you please explain this?

auditory hallucinations means when there is no one talking to him, patient percieves that some one is talking to him. so , what is actually happening? he is talking to himself and he percieves that some one else is talking to him! this is clear cut PROJECTION. however , I must admit that this is not my assumption.
