A 43-year-old DEAN JONES, is brought to the emergency room by his mother because she is afraid of him. He is wearing a wool coat, several sweaters, a wool cap, and mittens on the hottest day of the year. His affect ranges from anger to laughter. His speech and behavior are childish and when asked what is wrong, he replied, “The medicine is bad…tomorrow is mad”.

His mother also reports she has heard him talking to himself in his bedroom. When she questioned him, he denied that he was speaking, but then continued to speak when she left the room.
