Diagnosis of exclusion" this text is from Kaplan Pathology...so I think is highly unlikely for a 21year old person to have MCD.Plus the diagnosis is one of exclusion, that is why I wouldn't take into account an urinary cast for that diagnosis, at list not when it looks very much alike a red blood cast.http://courses.washington.edu/hubio562/urinaly/rf11.html

people...its easy... look at the cast first...can you see CELLS???? if you can see cells then its most probably a CELLULAR cast... cellular casts include RBC WBC epithelial cells RBC casts are redish brown, yellowish brown...there is always somethin brown with them this is a case of POST STREPTOCOCCAL glomerulonephritis... waxy casts dont have cellular element... try googling the casts images and learn them ...

Yep, those are most definitely red blood cell casts. They would have shown you the "maltese crosses" if they were fatty casts.
