Many medicine fellowships will not sponsor for an H1b visa. So, if you plan on doing a fellowship following residency, do your homework now and find out if places will continue to sponsor H1b visas.

Hi Mr.bbb and Others!!!!!

Well I need some help here,I wanted to go for USMLE, BUT the problem is that I did really Bad in my Pre-Clinical years,

Flunked in 1st Proffesinal Exam(2nd Year) and repeated the year. You do not actually take lectures etc just appear in the exam the next year.

Passed the 2nd Proffesinal Exam(3rd Year) First attempt,

Didnt appear first time in the 2nd Proffesional exam, and passed the second attempt (The attempt is different from repeating year,you just appear in the exam after a month or so after the first exam , called supply)

Didnt appear in Final Proffesinal Exam(2nd Year) and repeated the year,

I am not a DULL student but just lost my way due to some personal reasons, almost gave up Medicine. But after having gone through the med school ( I'm in Pakistan ) I recovered my sanity.

Did 1 year House Job (Internship) in Eye and got a Post-Grad Diploma in eye too passed it in the First attempt.

But as the USMLE requires Medical School Transcripts, I feel my chances are really slim even if I go on this path. I dont wanna be left in the wilderness without any Match after having done USMLE, I have already had my share of screw ups.

Please people can you comment or suggest anything to me. I wish I can turn the clock back but I guess some wishes can never come true sad and we can not undo the things we sometime do to ourselves.
BTW I do not want to make it into High Ranking Field/Speciality, as reality is otherwise BUT do I stand a chance of making it into ANY ?

What if I really put myself into it and try to get Good scores ( I know I know what you people must be thinking ) :wink: but lets just say if I do.
Please be as realistic as you can.

I can't lie and tell you that it won't matter, that PD's won't look at your transcripts, because they will. The best defense might be to address your performance in your personal statement. Explain that some of the difficulties related directly to personal issues, family issues; and how you rectified your study habits, personal life, etc.

Now - you know that you will have to STAR in the USMLE exams to totally turn the situation around. That will show PD's that if you apply yourself, that you can do the tests and get good scores - and that the period in your life when you were having academic difficulties is over.

Thank you very much for your advice, I really appreciate it.
Yes I know that it is not going to be easy but guess thats the way it is for most of us mortals. I will plan and device so that I can do good on the USMLE scores. When I go for the Step-1 and get the Score, I will know it whether I can make it or not.

What u said about johns Hopkins is not true..overall..i know some my friends whoare /were residents there..all IMGs and went on to do fellowships there..so they do take IMGS..but ofcourse u have to do be good !

I will take Step 1 in April, and I'm planning to take step 2 CK in July and step 2 CS probably in november. Is it too late for applying to a residency program starting in 2005?

Can I make a point clear which rarely comes up in this forum.

Scores are important, but having someone who knows you and gets your foot in the door is even more important. If your employer in Mozambique knows a Chair of Surgery ANYWHERE then use this.

The scores are often ignored when you actually interview (within reason). If the interviewer can phone a friend and get the 'real story' this will happen and makes MUCH more difference.

I am speaking from experience in sub-specialty surgery. If you are trying to rely on scores alone, you have a VERY tough job ahead of you.

This is simply reality.

I have a question for BBB (or whoever wants to reply)

I am an IMG (> 5 yr old grad.) working in a residency program but as a Medical Informatics Computer Programer and DO NOT need any visa sponsorship (currently hold a green card).
After graduation and working in a clinical enviornment for 2 years in my home country I got bored and to bring a little spice in my life & career I immigirated to USA, finished several Microsoft Certifications and bacame a computer Porgramer in Medical Informatics & Medicine related softwares.
Now after 3 years I want to get back into a residency (Anesthesiology/ Family Medicine/ Psychiatry....any of these..where ever available). I know I will be questioned for a recent US clinical experience which I do not have. Should I find an observership or an externship or Research???? What should I do to become digestable/ palatable for the residency programs once again?
I know having a 3 yr US HealthSystem experience under my belt could earn me some favor. On the daily basis I have to intereact with Residents, docs, health software programming etc which gives me a confidence of 'already in' but I know that being an old grad and no recent clinical experience could bleed me to death in the hands of any residency program folks.

What should I do to maximize my chances to be selected in a residency interview? Should I keep doing what ever I am doing till I get a residency? or should I go for a Research or an Observership or a weekend ER observership with my current job?

Not during your residency. Following training, include fellowship here, you will need to find an employer who will sponsor you for permanent residency or your green card (which are now pink). But it is following the training. You should start looking for the waiver position about half way through your training, giving about 18 months to 2 years to complete the paperwork process.

You are correct, with H1b, the visa holder has no obligation of returning to your home country. To obtain a H1B visa, you must have USMLE Steps 1, 2 and 3. You must have current ECFMG certification. And you must have the training license for the state that you will be working in. And you must have all this in your hands prior to submitting a complete application.

When does this visa expires?
You can remain in H1B status for a maximum of six years.

Is it difficult to change this visa later to permanent resident status ?
Not if you find an employer willing to sponsor you.

How large are costs of issuing this visa for Img ?
The J1 visa will cost you about $130 dollars. This doesn't count the cost of the exams and the CSA. The H1B doesn't cost as much - but most programs will strongly encourage you to pay for the premium processing. And it does work. And, a little known fact, it is written into the rules of the H1B visa, that the employer must pay for the fees for this visa.

since i am interested in doing residency in IM and do a fellowship in GI is it advisable to go to J1 insteadof H1.

can you please tell me which programme you exactly joined for cancer research. Are there any other progs of around 1-2 years duration which can be done after ecfmg certification or may be even just after step1 and step2CK and are related to surgery?

iam an img and would like to be a part of johns hopkins/mayo/washington/yale...........i mean the better universities.
will a score of 95 in both usmle-1/2 help,or do we need externship/observership........or any thing else.if required how can i get one ??//?/

these days, it seems everyone is scoring 99/99..your 95 is a great score, don't get me wrong, but according the the coordinator i met yesterday, it's so common now..as an IMG, you definitely need USCE, as many programs require that..good luck.

I have a simlar background as you, I am an old IMG, and I have worked as a programmer for 4 years and now I am ready to take the USMLE. May I ask you a question: How long willl it take for prapreing step 1 for a old IMG in general?

Hi Mr.bbb
I am a img from India. Presently I am on L-2 visa ( with my husband who is on L1 visa) and have a valid work permit for 2 years and I think i will be able to get it extended later. I have started preparing for usmle-part 1 and am planning to pass part 1, CS2 and CK2 by July, 2006 (if things work out as i have hoped) so I would be able to apply for the 2007 match. I have also got job recently as an ultrasound tech. Please guide me if it is possible to get residency on this visa with a work permit. And do i need to pass all the three steps before i can be accepted for residency ( as i beleived that we need to pass the part1 and 2 only to be able to apply and be accepted for residency). Also, will this experience as ultrasound tech be helpful when i apply for residency. I will greatly appreciate if you can guide me on this matter
