Just by process of exclusion, don't know the pathopharmacology:

Q says tachyarrythmia plus shock ...

So,
not A
Not B because in 3rd degree AV block rate is in 30
Not C Afib with rapid VR usually not in shock
Not E PSVT usually not in Shock

So D,

Is it due to long QT interval by methadone?

Methadone can cause QT prolongation, so I guess Torsades...
