A 43 yo man is evaluated for one year history of chronic abdominal pain . He describes episodes of epigastric and left upper quadrant pain that last for hours and not relieved by antacids . Certains food can precipitates the pain . He also complaints of occasional diarrhea . He has lost 10 pounds over the past 6 months . 4 years ago , he was hospitalized for 3 days with acute abdominal pain . He smokes 1 pack of cigarrette a day and consume alcohol regularly . His family history is significant for diabetes mellitus in his mother and prostate cancer in his father .

Which of the following is most likely to diagnose this patient 's condition ?

a ) D- Xylose absortion test

b ) serum amylase and lipase

c ) HIDA scan

d ) Ca 19-9 and CEA levels

e ) stool elaste

f ) upper GI endoscopy

g ) liver biopsy

I'm guessing D

-smoker, drinker, epigastric pain, diarrhea, weight loss.
--i'm thinking pancreatic Cancer/colon cancer?

(E)The answer is stool elastase, this patient has chronic pancreatitis and that test is the most specific and sensitive for exocrine insufficiency in chronic pancreatitis. He is at risk thought of developing pancreatic cancer.

F_ UPPER GIT ENDOSCOPY ( ERCP )

ERCP is the most datitisensitive and specific test to dx chronic pancreatitis

This was a confusing Q on UW, I got this wrong but elastase was the answer. I did not see that lab in step up to medicine or in the kaplan notes, only in the explanation to this q.
Good confusing Q!
