please help in this stepwise t/t

steps in management of patient with dog bites.

if dog rabid-- do we do kill dog immediately and its head sent for examination ??? OR first we need to give postexposure prophylxis to patient??

which step is first?:confused dog killing or immunisation pt??grin

1.-Provoked dog bite---------------->Observation of the dog (except if the bite is in the face: start inmunization but d/c if dog doesn't have any sign of rabies)

2.-unprovoked dog or bite from wild animals--------------->if animal available: examination of animal's brain/otherwise rabies prophylaxis is mandatory(Inmunoglobuline and vaccine).

*Tetanus prophylaxis and wound care are required for all bites.wink

Provoked dog bite:
-Provoked means you were somehow messing with the dog -either petting it or playing with it, and it bit you.
In this case, observe the animal -If it shows signs of rabies, immunize.
If the bite was on the face, then its best to start immunization and THEN observe. The brain is close to the face so its risky. If the dog doesn't show signs of rabies you could discontinue it.

-Unprovoked dog bite: This means a dog came out of nowhere and just bit you for no reason. (Same rules for wild animals)
--IF the animal is available, kill it and examine the brain for rabies -
--If the animal is not available, then rabies prophylaxis is needed (Ig + Vaccine)

If the wild dog is available, I think examine the brain first and if it shows signs of rabies, treat patient... Now in this case what is the treatment: Ig AND Vaccine, or just vaccine? or just Ig?
