10 years old child developed severe proteinuria. Urine dipstick is positive. He has swelling of feet and abdomen. What is his renin and aldosterone values from normal?

1. increased
2. decreased
3. normal

Normal.

increased levels is an attractive choice but hasnt been proved. so i wud say normal for Renin and slightly increased for aldosterone.

I sincerely do hope i m right cos i havent started this topic yet. but keep the discussion going.

my choice is increased....
icf shits to ecf due to decreased cop.... so decreased perfusion of kidney...
kidney thinks hypovolaemia.... starts producing renin wh increases aldosterone...
hope i`m correctsmiling facesmiling face

increase renin and aldosterone...
nephrotic--->hypoalbunimeia---->shift of fluid to ICF--->Hypovolemia--->stimulation of renin-angiotenisn-aldosterone system

there is no hypovolemia in nephrotic syndrome...ever heard of that??? grin

ICF fluid shifting isnt relevant...if you say its outside the VESSEL FLUID SHIFT then the person is normally hydrated...there is normal BP in the vessels of kidney...
always remember...
JG apparatus acts on fluid volume base...
if there is high amount of fluid passing through the renal tubule...it gives a negative signal to macula densa which gives signal to smooth muscles cells of afferent arterioles which produce renin...
intratubular fluid is normal unless there is renal failure...so renin is normal...

body controls blood pressure by NaCl...the normal SALT we eat...it doesnt have any other long term way to maintain that...proteins doesnt maintain blood pressure smiling face

even if we are severely hypoproteinemic as in kwashiorkor...there is no HIGH BLOOD PRESSURE smiling face use common sense...dont go to the intricate details of every mechanism...

I just gave this question....to let u know that ...its the way they check on boards smiling face best of luck

edema = increased fluid in interstitial fluid = decreased effective arterial blood vol =
decreased renal perfusion = increased renin n aldosterone.
