depends on how motivated ur to become a fellow..if u cant see urself as anything but a subspecialist in the future...J1 in a univ. based program is what almost secures that..on the other hand if u feel that a program has say a 50% fellow placement rate and is affiliated with something like JHU where u can get electives and u are confident of ur skills that u will work ur way to a good LOR from JHU electives(i am pathetic at that) and confident of ur research skills(another achilles spot for me), even a community program is good enough..but if ur goal is H1>GC only, u know the answer..as u remarked..there is no easy answer and needs a lot of soul searching..but remember.. H1>fellow from community program..happens all the time J1>fellow>practise in US...tough, now that waiver requirement is 5 years and waiver jobs are hard to get. If u are willing to go for J1, ensure that it is a very good program..i think that J1 is good for fellowship is a misconception from personal experience..Fellowship~ur performance in residency, institute from where u did ur residency, LORS, research aptitude..visa status figures towards the bottom of criteria for giving u a fellowship
