Cultures of synovial fluid are positive only in less than 40% of cases of purulent gonococcal arthritis. Blood cultures are almost always negative in patients with purulent gonococcal arthritis. Culture of the urethral swab is positive in around 80% of cases of purulent gonococcal arthritis. Urine culture is negative in patients with purulent gonococcal arthritis. Urine culture is of value when urinary tract infection is suspected and has no role in the diagnosis of suspected urethritis.
