phenytoin acts by blocking Na cond.. s/e gingival hyperplasia, hirsutism, hematologic, osteoporosis(GHOst) induces P450, teratogenic valproic acid acts by blocking Na and Ca chan...and inhibit GABA transaminase s/e hepatotoxic, pancreatitis, alopecia, inhibits P450, its least sedative, teratogenic please correct if wrong/ add if i miss

hi jayesh...phenytoin decrease the flux of NA in neurons it also decreases influx of CA during depolarization.... valproic acid....may " enhance" NOT decrease GABA action at inhibitory synapses... SE hepatic toxicity ( should monitor frequently... THROMBOCYTOPENIA AND INHIBITION OF PLATELET AGREGAION...so increase BT....

hi gul...i 'm not sure that i understand well the MOA of VALPROIC ACID i think that valproic " enhance" action of GABA.... gaba is an inhibitory neurotansmitter so if it's inhibits the GABA tansaminases then GABA increase...is it true???

GABA transaminase is involved in termination of action of GABA,so inhibiting it would have GABA-ergic effects.

thanks to all... one doubt it has been said that dantrolene has similar MOA to phenytoin . I doubt about it ( it was given in q bank kaplan)....anybody would like to clear this to me..thanks in advance

hi asmi... dantrolene acts " DIERCTLY " on muscles by interfering with the release of the CALCIUM from the sarcoplasmic reticulum. phenytoin stabilizes neuronal membranes to depolarization by decreasing the flux of NA in neurons during the resting state or during depolarization; it also reduces the INFLUX OF CALCIUM ions during depolarization and suppresses repetitive firing of neurons. hope this will help you... thanks a lot anne for your explanations.[/b]
