A 52-year-old male complains of progressive nausea, vomiting, and right upper quadrant pain for the past 5 days. He also noted a change in the color of his urine, with sleep pattern change, and confusion for the past 3 days. He suffers from chronic migraine and took about ten 500 mg acetaminophen tablets during his last attack of migraine 8 days ago. He is also a chronic alcoholic and has been drinking 2-3 drinks of whisky/day for a long time. On examination, he has tender hepatomegaly with asterixis. No stigmata of chronic liver disease are seen and the patient does not have any edema or ascites. His laboratory test results show Bilirubin 6.5 mg/dl, ALT 7000 IU/L, AST 8000 IU/L, PT 20sec, and albumin of 3.2 mg/dl. Which of the following is the most likely diagnosis in this patient? A.Acute alcoholic hepatitis. B.Acute liver failure. C.Fulminant hepatitis. D.Cirrhosis of liver. E.Wilson’s disease.
