So here goes :
1. Please tell me your name and age.
2. Where are you right now?
3. What is the date?
4. What is this object called? (holding my pen)
5. Please remember these three objects? (pen, cup, pencil)
repeat these please for me?
6. Spell World Backwards:
7. Tell me those three words please?
8. 100 - 7, minus 7
9. What are the last three presidents?
10. If there was a fire here what would you do?
Ok so my question is how do I score this? And what questions correlate to recall, registration, judgement, etc? How do we do the note for this thing?
Thanks SOOO much if anyone out there can answer this in a simple way.

5 points for time oriented: year, season, day of the week, date, month
5 points for place oriented: state, county, city, hospital, floor
3 points for each object repetition (right away)
3 points for each object remembered later on
5 points for "world" and serial sevens
2 points for naming an object or its color
1 point for repeating "no ifs, ands or buts"
3 points for obeying 3 stage command
1 point for obeying written command
1 point for writing a setence
1 point for copying overlapping pentagons

you don't have time for mini mental.
If you need this, what you will probably not, just ask pt's name, time and place and as max for that spell word backwards or subtract several 7th from 100.
