I was wondering what was peoples take on quality of program and success as private practioner or fellowship

1. Should one wait a year or so to get into good university based program

2. I know fellowship is greatly influenced by the program quality but what about working as hospitalist and/or private practice?

To get into fellowship, it's all about who you know. You will beat even university program graduates if u know the PD well enough. wink

I feel that I want to save a year and take whatever program offers me position.

I am unsure if I decide not to do fellowship, how would program' reputation affect my salary as hospitalist or as private practioner

As for hospitalist or practioner, there is not much salary difference in between MGH and St.Banabas(sorry for offending).
It depends on where you wanna practice. Generally, Hawaii>Coast>>Mid-West and Deep South.
But the program reputation does affect how many "clients" u ll be able to recruit if u:
1. practice in a place u didnt do residency nearby 2. U didnt do medical school in USA
This is the time dame comes into play.

Actually, Im not so sure it matters whether or not you got your degree in the USA. I worked with a doc once who told me that after RESIDENCY, patients dont give a hoot on where you went to med school. It's not like people ask about your credentials (other than what you specialized) when they come to see you.

Your salary, and success, depend more on geographic location and how WELL you relate with patients. If you're a good doc, people will spread the word and you will recruit more clients. If you're a bad doc who graduated from Yale, you wont get much because of your poor reputation.
