A 29 y.o. female presents with fever and sore throat and is diagnosed to have streptococcal pharyngitis. She s treated with ampicillin and recovers fully in 7 days. 2 weeks later, she again develops fever and also a rash and notices slightly decreased urinary output. Urinalysis: pH....6; specific gravity......1.022; 1+ protein; 1+ blood; no glucose and no ketones. Urine microscopic examination shows RBCs, WBCs ( including eosinphils) but no casts or cystals. d eosinophiluria is quite specifc for AIN (acute interstitial nephritis) time duration after the treatment also matches while post strept GN doesnt develop before 2 - 3 weeks until prior infections have been present other drugs that can do the same are aminoglycosides, penicillins, cephalosporins, sulphonamides and salicylates
