Pt with depression and starts hearing voices for ONE week -> MDD, schizoaffective, schizophrenia, bipolar.

I think for schizoaffective it has to be TWO WEEKS of psychotic symptos without mood symptoms therefore the DX could be Depression with psychotic features.

I think if he wants you to answer Bipolar there will be some thing referring to both mania and depression which is not here.
I go with"Depression with Psychotic Features"

DIGFAST: Distractibility, Imsomnia, grandiosity, flight of ideas, Activity(increased in goal oriented, Speech(pressured) and taking risk.
Yes, at least ONE WEEK of THREE OR MORE SYMPTOMS OF DIGFAST
