Usually hospitals have their own application form to fill out. If not then you write to observership coordinator that you are applying for observership with 1st, 2nd, 3rd choices of date and specialities you want to do your observership in.
Attach your CV with it to give personal contact info, usmle scores, ECFMg certified, Graduation school, year, country,any work experience in your country, visa status...pretty much like the CV in ERAS.
Check their website or call them to ask what all documents they require with the application and fees.
