you can use pneum otosc, you don't need a tympanometry to Dx OM

Tympanometry: Tympanometry can provide evidence of fluid behind the eardrum, while multifrequency tympanometry has become an accepted objective method to determine the status of the middle ear, especially in regard to diagnosis of effusion. some more information: http://www.emedicine.com/ped/topic1689.htm

Usually pneumatic otoscope is all that's necessary to diagnose Acute OM. If you see an IMMOBILE TM, its OM.
