A 52 yr old caucasian male complains of diminished energy. he says he feels sad, moves slowly. his wife died in a car accident 3 months ago and he thinks he is responsible for it coz he was driving dat time. he no longer has any interest in any of his favourite activities like fishing. he often has flashbacks of the incident. he complains of poor appetite.his has had a 20lbs weight gain. he feels like ending his life. wat should be the next appropriate step?

A. psychotherapy for PTSD

B. reassure him dat its normal bereavement.

C. prescribe fluoxetine.

D. order blood tests

E. admit for ECT

He has weight gain (20 lb), we should think about hypothyroidism. So order TSH and T3,T4 test first.

D. He had poor appetite yet gained 20 lb, so hypothyroidism should be ruled out.

Always rule out Hypothyroidism before treating depresion

comon people...at least I know this that we arent goin to rule out hypothyroidism for this case...its VERY CLEARLY RELATED TO A STRESSOR...whats the answer?

we always hv to rule out hypothyroidism in this patient coz of his 20lbs wt gain inspite of having a poor appetite.

UW says rule out hypothyroidism or for dat matter any other organic cause in such patients and thn proceed to treat their depression. smiling face

Why I always miss "weight gain part". I did this question like twice already. Maybe I need to ban my head on the wall to remember this!!!
