Finished exam in philli Quite a uqnie experience to say the least. Alright I am IMG, here for 7 years in USA and had one year of observership/extern in speciality clinic So English communication was not a big deal for me. I had to learn to communicate with patient about what I am going to do. Back home, I would straight go ahead and do exam, here you cant. Biggest challenge to me was that I was sick. Really sick with viral fever, vomiting and diarrhea two days before exam. Then, I had to drive to philly day before. I thought of cancelling but did not wanted to ruin the chance of matching this year. Dates are sparsely available. Tyenolol was my best friend. Reached to sheraton day before..found great rate on hotwire.com. Nice hotel, 4 blocks from train station on 2nd street. take a train to 34th street and exam center is two blocks away. My concern was health. I had spent three hours reading sitting on the lavetory seat. But I had resolved, I wont quit. Morning I woke up, had a shower and I felt little better. Only to walk in the cold wind outside and feeling hungry and nauseated again. And in all hassle to be on time, forgot to take my medicine. Reached to exam center and found a little deli in the building next to the exam center. Most examinees were american students and could be seen in the store. Young chinese owner was very helpful, he gave me his own tyenolol as there was no pharmacy around. Once in ceter, everything was smooth and clean. Overall exam, was little difficult then I expected. Few cases were different then what I had seen in FA. Cant discolose them due to their policy. NOt sure if they were experiemental ones. Idea is to keep mind open and look at all possibility. Managing time was bit difficult. First five cases, I did not finish two of those. Could not give out differential or educate on testing. In break, I asked myself what am I doing wrong. All the time, I practiced, I had two minutes to spare. GUYS and GALS, difference was SPs take time to answer. Our friends already know the answer and move fast. These guys take long.... pauses. Some actually took long time to even eloborate. there was no way I could do all exams not even all pertinent one. So I changed stratergy. I was allready feeling little better with adrenaline pumping through. I then opted to be very focused on cheif complain, do only vital exam for diagnosis and then offer counselling. Thing were better now. Next four cases were simple FA type. now most important thing, even in the hospital I work, patients dont were gown in outpatient setting. I wanted to not examine through gown. I had three females examinees in which I did terrible mistake. TO EXAMINE BELLY, I ASKED THEM TO LOWER THE GOWN FROM TOP ON. EXPOSING THEIR BREAST. EVEN THOUGH YOU WANT TO DO RS/CVS EXAM NOT A GOOD IDEA FOR ABDOMEN. ASK THEM TO PULL IT FROM BENEATH. I did not realize this untill SP herself started doing the right thing. I always thought that it would be uncomforting to pull it from downwards as that would expose their underware. I forgot about covering that they provide. so i had major screw up. I am not sure how many points would I loose for over exposure. I could manage the rest of the exam. Ofcourse, I forgot or could not do some exam that I should have but I finished in time, had majority of information I should have. finger crossed, still little sick but hey I DID the exam.
