Women suspected of having PE, the V/Q scan was negative what do you do next? Spiral CT, d-dimer, CXR, ekg (angiography wasn’t there)

D dimer if negative no PE
On ECG you can find S1 Q3 and inverted T wave in lead 3
Spiral Ct is very tempting.

Step-up to Medicine says:
A normal V/Q scan virtually rules out PE-no further testing needed.

since suspected of having a PE we need a test to rule in rather than rule out PE. d dimer can rule out but cant rule in as its not that specific. Spiral CT would be my pick.

whats the ans?

When the V/Q findings and the clinical suspiscion of PE are discordant , FUTHER testing should be performed .

In this case , since Pulmonary Angiography was not there , the best answer should be SPIRAL CT. The other options (EKG , D-dimers ) are NON DEFINITIVE TEST
