"1st Nasotracheal intubation " is ok "airway it the 1st of ABC" and any neurotrauma patient with GCS of 8 or less requires intubation but 2nd and 3rd is a dilemma , too short case, is there is signs of intra abodominal bleeding ? as shock maybe from other causes like pelvic fracture >>with penetrating injuries that will require surgery anyway, start with surgical intervention then the fluids >>if we dont know where the bleeding is coming from or it might stop spontaneously or not start with fluids then if the patient is hemodynamicly stable go for CT scan if unstable do diagnostic peritoneal lavage or sonogram in ER or in operation room

Not enough info in stem. But to get you started in the scenario you gave ABC BP is too low ... open up too large big bore IVs then in the stem they will tell you .. abd or pelvic trauma or whatever. Then you work them up from there.
