I do not have surgery books so go mahead an talk about ALL the leg pediatric stuff.

Do you have the Kap Peds Book? its all in the ortho chapter there too..

Well anyway, here's a quick review -I like to think of them by age groups (it helps)

Newborn -
1. Clubfoot (Talipies Equinovarus) Tr: serial casting and surgery
2. Developmental Dysplasia of the Hip -Easily dislocated with a jerk and click and brought back to normal with a snap. NO X-Ray because hip is not yet calcified. DO U/S -it's diagnostic. Tr: Abduction splinting with a Pavlik harness for approx. 6 months.

*Note: Hip pain can show up as Knee pain, but NOT the other way aruond.

Toddler -
Septic Hip
Its an EMERGENCY
Usually after a febrile illness, and the baby refuses to move the hip.
Increased ESR, DX: aspirate the hip and open drainage if pus is obtained

4-12 year olds -
Legg-Calve-Perthes aka Legg-Perthes aka Avascular Necrosis of the Capital Femoral Epiphysis (and I hope there are no other names for it!)
-like the name suggests: it's avascular necrosis of the femoral head
-insidious onset, pain, limping, and decreased motion.
-worsened by moving, better with rest
-Dx: AP and lateral hip or frog leg X-ray.
-Tr: Controversial... but they say Maintain joint mobility,
According to UW: if <5y/o: NO treatment, and if >5y/o: Abduction bracing or Surgical Corretion.

Teenager -
1. Slipped capital femoral epiphysis: (think of an obese or lanky teen)
-An emergency
-groin or knee pain
-limited hip motion
-Dx: X-Ray
Tr: surgical -pin the head back in place

2. Osgood Schlatter disease (Osteochondrosis if the tibial tubercle)
-as the name suggests: pain over the tibial tubercle
-Tr: immobilize with an extension or cylinder cast

3. Scoliosis
-adolescent girls -more common
-spine is curved
-look from behind when she bends over and you will notice a hump over one side of the thorax
-Tr: Bracing -will help arrest progression; surgery only in severe cases
