loss of normal negative charge of GBM and selective proteinuria(albumin)----->loss of oncotic pressure...collectively known as nephrotic syndrome

there is basement membrane thickening in diabetic nephropathy!

so what happens as a result of it? :?: and by what mechanism of action ?

Is Basement membrane thickening the earliest change in DM nephropathy? Then there's increase in mesangial matrix resulting in either diffuse or nodular glomerulosclerosis. mechanism may due to advanced glycosylation end product (AGE). please corrent me if I am wrong
