Really I don't know what to write. Anybody who took exam can advise? The question remains the same - are we writing what we actually see( we are dealing with normal person so we don;t see much) or we are writing what we would expect? Example would be increased tactile fremitus, dullness to percussion and some other symptoms that patient can not simulate. Thank you for suggestions

Write what you see, not what you would expect to see. If the SP has something real, like an unrelated scar on his abdomen from a real apendicectomy, ask him about it during the physical exam and write it down too, but don't get distracted from the focused physical examination. If the SP is simulating something for you, it will be obvious, and you assume it is a real sign or symptom.

i remembered a normal PE note and edited it from there. it helped me stay calm while i was typing my PN. but always write was you saw and noted. i mean if the SP was coughing, and in pain, then note that. don't say Clear to Ascultation bilaterally. and start with vital signs, even if you write WNL (Within in Normal Limits). and memorize the abbreviations, although there is a list in each cubicle.

Go through First Aid for CS and look through all the cases and PN's...I think that is the best book for CS...
