A leukemia patient who has undergone multiple courses of chemotherapy develops herpes simplex encephalitis. Which of the following would you expect a CT scan of the patient's brain to show? A. Generalized volume loss B. Volume loss selectively in the basal ganglia C. Volume loss selectively in the brainstem D. Volume loss selectively in the cerebellum E. Volume loss selectively in the temporal and frontal lobes The is tricky so think before u answer. And plz be kind enuf to explain ur answer.

it is e. hsv almost always affects the temporal lobes and it is common in immunocompromised aka leukemia with chemo patients

HSV encephalitis involves temporal and frontal lobes, which is E. But there is something fishy about this Qs, otherwise you would not post it here, NNL, am I right? So what is the answer. Not sure how multiple chemotherapies affect the CNS. Could it be A, general volume lost?

Dont feel lost you will face a lot of questions in the real exam in which you will be going is it this or is it that but honestly in teh end you should always go with your guts and feelings because of the times you will be correct i.e more than 75% of the times you will be right.

The correct answer is E. Herpes simplex can cause a necrotizing, hemorrhagic acute encephalitis that may rapidly produce death. The encephalitis characteristically involves the lower portions of the cerebral cortex, notably the temporal lobes and the base of the frontal lobes, possibly because the infection spreads from the oropharynx.

  E , thank god it is rt GG ........ as NNL said go with ur gut , do not think too deep, common things r common & also , do not doupt self & cahnge the answer it is my habit , bad bad habits changing the answer or choose answer taht I thought not rt by trying to avoid 1st answer in mind   Anyhow, thnx for qs , it reinforce the knowlege , is taht from which Qs if u do not mind NNL or u can jsut say .. it is Not fr any NBME , thnx . since all kinds of ecephalitis can affect cortical and subcortical brain tissue it could be A and because, since the course of infection is not mentioned here, but if its from oropharynx as you mentioned then E is true

Is the volume loss caused by the destruction of involved lobes? Does HSV also cause calcification of the involved areas?
