A 34 year old woman gravida2 para2 comes to the office for routine visit,6 weeks after the delivery of a healthy baby.She states that she is feeling well&has just gone back to her work as physician's assistant.Her pregnancy was complicated by gestational diabetes controlled with insulin.She had a normal vaginal delivery.She is currently taking no medications&has no drug allergies.A 75 g ,2hour oral glucose tolerance test is performed on the patient at this 6-week postpartum visit& it shows

Fasting glucose - 150mg/dl
2 hr plasma glucose - 210mg/dl

which of the following is the most likely diagnosis?

A.Diabetes mellitus.
B.Impaired fasting glucose
C.Impaired glucose tolerance
D.Persistent gestational diabetes
E.Resolved carbohydrate intolerance.

so what is the answer?

A is the answer.
This patient has elevated fasting & 2hr value.so the diagnosis is DM.

B is incorrect b'coz this patient has a degree of glycemia that goes beyond impaired fasting glucose which is defined as a fasting plasma glucose of 110 to 125 mg/dl.

C is incorrect b'coz-impaired glucose intolerance is defined as a 2hr glucose tolerance from 140 to 199mg/dl.

D is wrong as there is no such term called persistent gestational diabetes.

E.This pt certainly does not have resolved carbohydrate intolerance.

Now my question is:
A patient with Gestational Diabetes, you need to wait for how long for her Glucose to come to ormal levels?

Also number 4 is new for me never heard of it.

ya i go with A too.

is there any term called persistent gestational DM? as far as i know, the gestational diabetes starts correcting itself as soon as the pt delivers.isnt dat y we have start a glucose drip in these patients during delivery, since they will b having an excess of insulin once they have delivered and can go in hypoglcemia?

these patients are at more risk of DM 5 to 10 yrs after delivery.
