tet spells occur due to increase the shunt from Rt.vent. to the over riding aorta through the VSD, any couse that will increase this shunt will lead to tet spells ; like dehydration , infection, exercise , tacycardia.
and to treat it you play the same principle , Fluids ,Beta Blockers , increase TPR and Venous return.
This is according to my understanding.
