1.renal vascular hyperplasia is due to which of the following
a.aldosterone
b.renin
c.angiotensionogen 1
d.angiotensionogen 2
e.angiotensionogen 3
could you please explain why?

2.lady has 90% of left renal artery and 60% of right what do you do?
a.stent both
b.stent the left renal a
c.stent the right renal a
why?

no 1 got any ideas i do have an opinion about the second question but waiting for some replies and then i will let you guys know what you think.
So please do answer your thoughts and opinions

For the 2nd Q is this all the info u have ?

It will depend on the renal function ,any lab values (Cr) too. With 90% involement we have to go with stent for the left renal artery.

i will pick b for this Q.

yes thats what i think also u stent the 90% stenosis

for first i don't know i think renin which cause arteriole vasoconstriction beside convering angiotensin so it may be renin

for 2nd iam with b

if u r going to do a stent, why not to stent both??

good Qs......
for the 1st.....i think it is angiotensin 2....it cannot be renin.
we give patients either ACE inhibitors or angiotensin receptor blockers, to control renovascular hypertension and to prevent renal hyperplasia.
so rennin cannot be the answer.

for 2nd.......i think it has to be....the artery with less degree of stenosis that is 60 percent.
the reason being....there are more chances of the kidney with lesser degree of stenosis or kidney with better perfusion to survive long....so always protect the kidney which has better chance of survival....

u go for stenting the kidney with 90 percent stenosis u land up losing the better functioning kidney...as even it wud later on require stenting, but then both kidneys have together have only 20 percent of capacity.

some more info.......as far as i remember this rule doesnot apply in case of renal transplantation.....and we donot change the better kidney....as there are very less chance of acceptance.......we gonna lose the only and better kidney...
