Can we apply for transitional year instead of prelim before going in for neurology in the advanced year? It would be great, if someone can throw light on this. Thanks a bunch!

yes you can...altough it is much more competitive that IM prelim, less work and less positions.
competitivity:
Transitional > preliminary IM > preliminary surgery

The American Board of Psychiatry and Neurology (ABPN) requires that the PGY1 year be spent in an accredited medical training program. The Residency Review Committee for Neurology and the Accreditation Council of Graduate Medical Education (ACGME) specify that 12 months of general internal medicine training is ideal prior to entry into the three-year neurology residency. This year must include at least eight months of direct patient care in internal medicine, no more than two in emergency medicine, and no more than two in neurology

I don't thnk a transitional year will satisfy this requirement... you need a prelim IM year to get the 8 month of IM you need for advanced nuero placement...

Besides as the above post atates prelim is easier to attain than transitional...

Our trans programs take Neuro residents, but then they work with me for 8-9 months.raised eyebrow

Yes you can do transitional instead of preliminary, I specifically know a Neuro program that has an agreement with the Trans program at the same institution. Before having the Trans program, the agreement was with the preliminary IM at the same institution. Needless to say, the bulk of the work is done in the same IM program as before.

One rationale for a Transitional program is the possibility to choose electives based on your secondary program interest, otherwise preliminary can be almost exclusively IM.
