23. A 27-year-old man is brought to the hospital byfamily members because he has remained in his room for 3 days. Hehas refused to go to work or eat with the family, and he expresses concernthat family or friends may try to kill him. One week ago, he wasdespondent when his girlfriend of 5 years abruptly ended theirrelationship. He has no history of psychiatric illness. Which of the followingis the most likely diagnosis?A) Adjustment disorder with depressed moodB) Bipolar disorderC) Brief psychotic disorderD) Dysthymic disorderE) Schizoaffective disorder

Well I think that He is having a paranoid delusion after an stressful situation, therefore i will go with Brief psychotic disorder which can be diagnosed if the symptoms last more than one day but less than 1 month and it can be 1 or more of the following: 1. hallucinations 2.-Delusions 3.-disorganized speech 4.-disorganized or catatonic behavior

I would go with A. adjustment disorder. " ... expresses concern that family or friends may try to kill him" is not psychotic. He's just been dumped, feels worthless/depressed, and possibly feel that his family might just finish him off (which is kind of delusional, but is mood-congruent).

I think (A) Adjustment disorder with depressed mood since there is no hallucination nor peculiar behavior , so brief pyschotic behavior is excluded

I agree it probably is A, but here is my thinking: Delusion definition: False belief not shared by other. For example: The feeling of being followed by the FBI. In this case the patient is concerned that family or friend may try to kill him. I think that is false belief or does it make sense? Some light pz

Yes, I think I see where Ivonne is going. But with delusions, there is false ones (eg being loved at a distance, followed by FBI), which could happen in real-life but is very unlikely. And then there is bizarre ones (eg ideas of reference, thought insertion/withdrawal/broadcast), which if present is considered 'psychotic', and would deserve a diagnosis of schizophrenia. In this case, you would need more than just 'I think my family is trying to finish me off' to be diagnosed as brief psychotic disorder.
