please answer anyone

if gastric ulcer with H.pylori is present
what would be the treatment

is it ranitidine or against H.pylori
please answer exam is near

for H.pylori?
and we can do H.pyori breath test or fecal antigen test to confirm its erradication after 2 weeks of rx.

so v have to give ranitidine to this pt? and then against h.pylori

When you are suspecting Duodenal Ulcer in pt. you should start in empiric treatment including antibiotic because DU coexist H. pylori infection.

Gastric Ulcer that had been confirm presence of H. pylori also need to start three combi meds.

you can combine AMOXICILLINE, CLARITHROMYCIN, OMEPRAZOLE (PPI)
or choose less expensive treatment with 2 antibiotics between Erythromycin, Ampicillin, Amoxicillin, Tetracycline, Metronidazole and add one PPI.

Ranitidine could be given but PPI is more effective.

Faecal antigen test for H.pylori???? Interesting----Can anyone explain on that?

To diagnose H. pylori--- Serology (sensitive) and Biopsy (specific)
To monitor treatment--- Breath test and Stool Ag test

However if you have a patient with dyspepsia <45y/o and NO alarm symptoms, you can use one of the non-invasive tests for H.pylori (Serology or Breath Test)
If the test is Positive--> Start AB Therapy (PPI, Amoxicillin, Clarithro)
If the test is Negative --> Start PPI, H2 Blockers, or Prokinetic

Use of H2 blocker gives you a 50% healing
Use of a Proton pump inhibitor gives you a 80% healing

Remember that smoking will exarcebate the ulcer and will most likely recur.
so Like Fara says advise of stop smoking, to everyonegrin
