She is withdrawn, irritable, fearful and restless. She reports feeling depressed for about 6 weeks, with difficulty falling and staying asleep and 15 lb wt loss due to loss of appetite. She has suicidal ideation with a plan to take overdose, which she had done in the post under the influence of command auditory hallucinations. She hears voices pretty much continuously, but when she is not depressed,they do not talk of suicide. She has been divorced for many years and has trouble holdingjobs due to her suspiciousness and odd responsed to other people 2) 35 yr female who wants her primary care physician to evaluate her for chronic fatigue syndrome. She doesnt have a history of viral or flu-like symptoms, just intense fatigue, hypersomnia (sleeping upto 11 hrs oer night). 10 lb wt gain, and generally low mood. She says she can feel better briefly when she is out with friends, but mostly she is sad, discouraged to the point of hopelessness about becoming her normal self again, and worthless, because she is not the person she wants to be. While she says she would not intentionally hurt herself, she would be glad to 'just disappear', if she could. She drinks socailly, up to two beers every other weekend 3)50 yr man who feels sad and confused since being laid off from job 2 months ago. He cant seem to get himself to look aggressively for another one, saying he is too old and no one will hire him. He sleeps ok, except on nights before job interviews, but he feels tired and dragged down many days. He drinks a beer with dinner every third night. Occasionally, he has bouts of restlessness and pacing, though he does not describe full panic attacls. He has some thoughts that life is not worht living, but none of hurting himself 4)64 yr female with 5 mo history of depressed mood, markedly decreased sleep due to early morning awakening, 20 lb wt loss, crying spells and instensifying suicide ideation. She says mornings are the hardest time, she feels so guilty about not getting up and doing all things she should. After grieving the loss of her husband, who died 3 years ago, she became active in church groups and volunteer work. Now she just goes to work and come home, since nothing feels worthwhile or enjoyable. She denies alcohol use and has no major medical problems except hypercholesterolemia managed with diet.
