A 35-year-old man comes to the physician because of daily severe, periorbital, right-sided headaches over the past 6 weeks. The pain often awakens him from sleep and is so excruciating that he wants to bang his head against the wall. He reports nasal "stuffiness" and nausea. He has had similar episodes on 2 previous occasions. Physical examination shows right-sided ptosis, lacrimation, and reddening of the right eye. Laboratory studies show:
Hemoglobin....................................16 g/dL
Hematocrit.....................................49%
Leukocyte count.............................6000/mm3
Erythrocyte sedimentation rate.........5 mm/hr
Which of the following is the most likely diagnosis?

A. Classic migraine
B. Cluster headache
C. Common migraine
D. Subarachnoid hemorrhage
E. Temporal arteritis
F. Tension headache

periorbital ,previous episodes( occurs in clusters)ptosis ,lacrimation

Cluster
daily, one sided, excriciating, red eye, lacrimation -all point to Cluster HA.

Migraine would also be one sided but without the other signs.
It could occur with or without aura.

(Whats the difference between CLASSIC Migraine and COMMON Migraine)???

Worst headache of my life SAH

periorbital pain during night Cluster

Band like headache Tension vascular headache

Headache associated with blurring of vision,Neural symptoms Migraine

Morning headache with vomiting, Raised Intracranial pressure
