out patient-- first MARCOLIDE--ERYTHROMYCIN, OR alternative FQ levofloxacin,gatifloxacin. inpatient-- FQ(gatifloxacin,levofloxacin)+ macrolide(erythromycine) + cephalosporin( ceftriaxone and cefipime)
