H. Increased Estrogen from the HRT she is taking. The question asks which answer can explain the findings, including the decrease in LIBIDO, her reason for coming in; Not the ruguful vagina. If the vagina was dry and atrophic, then the answer would have been a decrease in estrogen from the lack of ovaries.

After menopause, the ovaries and NOT the adrenals become the most important source for Androgens which are converted to E3. This female had a TAHBSO therefore loss of both androgens and estrogens. But the replacement of Estrogen gave a positive effect on her vagina but it also increases SHBG, which in turn decreases the amount of FREE and available testosterone, which is assumed to be responsible for libido and sex drive.

A decrease in androgens by itself cannot explain the findings with the vagina.
