pt is a 53 y/o male present with new ulcer on his right lower extremity. Pt past medical history consists of HTN, DM, GERD, CAD. after dressing the would and place pt on oral antibiotics, you noticed that ulcer is secondary to DM neuropathy. patient also has pain in his leg. he asks you to give him some pain medication. what's the best in management? a) NSAID b) Gabapentin c) amitryptyline (FIXED) d) tight blood sugar control e) Phycial therapy f) Tylenol extra-strenght g) Reassure

B painful Neuropathic ulcer in DM can be treated with amitryptyline,phenytoin,carbamazepine,topical capsaicin or gabapentin

C. Gabapentin is not helpful to control the pain. Correct me if I'm wrong but is more usefull to prevent neuropathic pain.

the answer is B............... you don't wanna give TCA to patient with history of CAD....ELITOKI, Gabapentin is an alternated drug after TCA to treat DM neuropathy
