After chemotherapy with doxorubicin ( adriomycin), there is an irreversible effect upon left-ventricular contractility and ejection fraction.
what is the most accurate method of assessing the effect of drug upon the patient's condition?

A. Transthoracic echo
B. Transesophageal echo
C. Left heart catheterization
D. Right heart catheterization ( Swan-Ganz)
E. Nuclear ventriculogram ( MUGA)

well for EF % MUGA scan is best option .... but along with EF here left heart contraction is to b checked so can b done trans thoracic echo too....

confused between A and E

The question is about the most accurate test for the diagnosis of Doxorubicin effect on the heart ...i.e.dilated cardiomyopathy is A i.e. TRANSTHORACIC ECHO......It will show dilated chambers and thin stretched out walls...

MUGA scan is only to check the ejection fraction which can b reduced in a nmumber of cases...

E. Nuclear ventriculogram (MUGA)

Explanation:

The most accurate method of assessing the ejection fraction is the nuclear ventriculogram, or MUGA scan. This technique assesses ejection fraction by determining how much of a nuclear isotope is ejected from the ventricle with each beat. It is not based on a two-dimensional, cross-sectional image and extrapolation, such as in the case of an echocardiogram or traditional contrast ventriculography by catheterization. Swan-Ganz catheters measure cardiac output, not the ejection fraction: It is the only method listed in the answer choices that doesn't actually measure ejection fraction at all.

shockedi just checked kaplans notes too and its written that echo is the KEY DIAGNOSTIC STUDY FOR DILATED CMP......how can we know on the exam that they are asking us what?

yep.
but reading the qs again - "what is the most accurate method of assessing left-ventricular contractility and ejection fraction" - will be nuclear ventriculogram
