In post MI complications,
UW says papillary musle rupture is ass with regurgitant murmur with radiation
KAPLAN says ventricular septal rupture is ass with regurgitant murmur and radiation
which is correct........guys iam confused

i think so both are correct .... see it depends on which wall infarction is there .... if septum is involved in infarction there willl b rupture of septum ( very rarely ) and it causes MR or TR due to damage to mitral or tricuspid valve.....

if inf wall infarction papillary muscle which holds chordie tendinae and mitral valve will cause MR

in case of ant walll infarction ( right ventricle ) TR willl b post MI complication

please correct me if i m wrong ....

yeh ur right........i agree.......but i remember a Q from UW Giving infarction involving anterior wall,regurgitant murmur with radiation
and the answer given was papillary musle rupture.
so please explain

actually
anterior infarcts .......leads to septal rupture[v1,2,3] and left ventricular rupture[v2,3,4].
inferior wall infarcts.........leads to right ventricular infarction andpapillary muscle rupture

ohhhhhh !!!!! there must b some explanation to it ....sorry i cant figure it out .... logicallly i still think ant wall infarct willl have chordea rupture , Right ventricular rupture ... not left as ant wall is made of right ventricle

please can anyone solve this issue.....
