it could be due to shoddy medical work. we think it's unlikely to happen in the us, but even that rare incident has to happen sometime. [perhaps if it had happened elsewhere, it would be more believable. say, in a developing country.]

why would they start with the face. as far as i remember autopsy is started with a Y incision in the torso. i may be wrong, or there might be other indications for starting with the face
