That is up to you. You decide by yourself when you want to take a break. I personally did not take any breaks at all during the two days (only to go to the bathroom).

You can take it after every case end.
In my exam, CCS finished early (10 min), so I did 4 cases-->break-->next 5 cases-->exam end.
