I wanted to ask you about my chances of getting into a good University program:

Creds: 94/96/CS passed/ Step 3 will take in Nov
Additions: half a dozen International and National presentations
Two grants from the Federal Government of my country
Research Studentship from the apex body on research in my country
1 month of observership in US with LORs

My question is... When is it too late to apply to programs? ( considering the respective deadlines of programs, of course). Do program directors review all the applications they receive until their deadline? I have applied to a good number of programs but I do wish to apply to a few more. Is it too late to apply now?

Dear Dr. Fischer,

Its an honor to have you on the forum.
I want to know:

1. If we apply to 2 different specialties IM and FM in the same program, and are lucky enough to get both interview calls....How bad does it look ?

2. What would be the best answer if someone in interview asks " How would you explain your applying to 2 different specialities here? "

3. If I go for an interview to a program which offers prematch and this is the program I absolutely want to be in coz of geographic location and husbands job.
How much interest can I show them and say what to convey it, without/with sounding desperate ?

Greatly appreciate your help and support.

Dear Dr. Fischer,

Hi. I am super happy and excited that I can ask my questions directly from you. Actually, I still doubt that it would be really you!

I am an IMG graduated in 2003. From that time, I have two years of experience as a GP besides wide research experience all at University and in Academic atmosphere from my own country (with seven ISI publications).
Very soon after, I pursued as a MSc student (Molecular Biology) in Canada which brought me two more significant publications. But unfortunately, I did not have time to pass USMLE exams (which I have planned to them in coming year).

I have following two questions. I will be so much grateful if you kindly answer my questions. I have lots of worries about them.

1. Considering the year of my graduation (2003) from MED SCHOOL and my engagement in research, and the point that I can not make good grades in USMLE exams if I want to reach Residency Match 2009. Will I still have chance for Match 2010 if I make good grades on exams? Which one is better, having high scores in expense of getting one year more of graduation gap, or finish the exams ASAP?

2. Is having PhD degree has positive effect while it will end in more graduation gaps? What about involving in PhD degree? Will it be interpreted that this I have not absolutely planned for Medical Residency?

I will highly appreciate if you kindly consider my emotional stresses and help me to make an appropriate decision.

Nice to hear ur replies which are very informative,

I'm an IMG,step 1 87/step 2 95/cs passed/all first attemp/ecfmg certified in sept last week/currently doing 1month USCE in university hospital/Applied for step 3,yet to block exam date//////Applied to IM programs in sept 13
One interview so far.

Here's my question,
Wot's more important step 3 or observership for my credentials?????!!!

I got a chace to do two months observership in neurology which is not my interest field from nov to dec,,,

Or shall I start studying for step 3 dropping the obeservership???!

Dear Dr Fischer,

it is wonderfull to have u here answering our questions, Ive matched the last year in a community program, thats considered a small one, though I see it a wonderfull program now.

I feel from your answers that scores really matter, but from what I see and know that many many applicants score more than 240-250 on both steps now, that was real in the last match, and I think scores are getting higher every year, then how do u think score do really matter.

I have high scores, but I believe many other factors do matter, most of them can't be shown on CV or documents, what do u think?

I am looking forward to seeing your anwer on that question because I am an old IMG too.

This is my background:

Independant Chinese lady

A graduate with M.D. in 1988. MS. and Ph.D. in 1966 and 2000 in China, respectively.

Have been working in internal medicine ( clinical practice and teaching duty) in colleage hospital before Sep. 2000 when I started to do research related to medicine in US. ( Academic title- postdoctor researcher).

Have papers published here.
Currently, hold H1-B1 visa and EAD, I-485 pending 4 months.
No USCE so far because I have to keep the job until my I-485 (NIW) is approved.

Plan to participate in Match 2009.

USMLE exam score:
Step I---229/95
Step II CKand CS will take next year

I don't want to give up though I know I am an old IMG.

But, I really want to know " Why are programs so reluctant to take older grads ?"
I wish I be treated fairly in next Match season.

By the way, my speciality was cardiology in China. In particular, my M.S. project was related to Cardic-electrophysiology. Ph. D. project was relatd to the Hypertension. Have ever participated several mutile-center, double blind clinical trials.

My ongoing scientific project is to study the cell electrophysiology with whole cell patch clamp and molecular techniques.

I hope all of above training and education background and experience can help me to reach another goal- to get the clinical training chance as a residency in US.

Dear Dr. Fisher;

I am an older IMG (1995). I have received a request from an IM program coordinator for further consideration to their residency program.

They need a letter from a faculty member regarding my IM rotations at school. As you can imagine after 12 years, it is near to impossible for me to find a faculty member who would remember my performance and therefore provide a letter.

How should I reply back to PD's request so he/she does not hit reject button?

Buenas Dias!! Good Day!! and relief of Anxiety to you all

1. Older graduates are considered less desireable by SOME because people are felt to be set intheir ways, and cannot be easily changed. There are a few places, however, that consider it an advantage, but they are fewer

2 Step 3 with a good grade is INFINITELY better than an observership.
3. Some places DO ask medical questions on interview. There is NOTHING you can do to prepare for this shocking lack of good manners. Also, any place that does that, is so nervous about its applicant pool that they have to put in this extra screen. run away if you can!!!!

It is in poor taste.

4. Can you apply to TWO specialties in the same hospital.
Yes! but it will be embarrassingif they find out. THis will be VERY unlikely.
Can you date two women in the same house
Yes! but it will be embarrassing if they find out. This is very LIKELY

I am nig fan of yours , sir one of friends not suppose to give any sort of the usmle exams just saw your lecture for one hour. He was soo much impressed that he saw all the lecture though he had not to prepare for teh exams and he was a MD IM in my country

Dear Dr. Fischer,
Appreciate your great work, teaching style and immense interest in healing the sick.

I once read " A great teacher is like a spark plug, not a fuel line." He makes you wonder, discover and realize the subject not feed you information. I have found that great teacher in you.

I am IMG graduated in 1999. Did PhD at Georgetown Univesity in 2005. Now I am a junior faculty. I have step 1 score of 93, done with CS and writing step 2 ck soon. I have few publications and one year of observership experience.

I am considered "old" IMG by many programs, while my co-workers in MD/PhD programs are not as their graduation year would be like 2005. How can I improve my chances of match in university based program.
Your time and advise is invaluable.

I am IMG .I did my graduation with Honours and Gold medal in Medicine.
Then i got opportunity to train in UK in Psych.
I have 234/95 step 1/ 234/97 Step 2 /Cs Cleared/2003 grad
With my Psychiatry experience what Q an I expect at the interview??
