Can you tell us what will be new in the videos? will the entire internal medicine will be you teaching? A friend of mine told me there was another MD teaching in the videos, will he return? what is the best way t learn the management and algorithm? thanks

Hi Dr. Fischer, Thanks for taking the time to answer our questions. I took the Kaplan video course in 2006, and it was a pleasure listening to your lectures. Anyway, I was wondering about the following: For CPR, the 2005 notes say "15 compressions and 2 ventilations, regardless of whether one or two rescuers is present" BUT in lecture it was mentioned: "1 rescuer: 15 compressions to 2 Breaths" and "if 2 rescuers: 5 compressions per 2 breaths" However since then the AHA has changed the guidelines and now they say: "1 rescuer: 30 compressions to 2 breaths" -adults, children, and infants. and for 2 rescuers -I'm not sure... Has it also changed or remained the same (5:2)? And it's questions like this which bring up a big frustration for me... I read something in the Kaplan Notes, and then read something contradictory elsewhere like USMLE World, or in another textbook, and then agonize on which source is the more accurate or up to date. And if something is very recent information, and I know it's right, then I wonder if the USMLE can test something so recent. Can you please advise on how to overcome this problem. First of all, how would I know when something in the Kaplan notes is out of date? And how can I find the most recent information which is fair-game on the USMLE so I can keep my knowledge up to date, but at the same time not get overwhelmed with useless information? Also, if some guideline changes today, when can the USMLE test it -after how long? Thanks in advance. DrVirgo

Hi Dr Fischher...this what you are doing here is soo kind and humble of you and a simple "thanx" is not enough to express it...still ....THANX what I wanna ask is... there is no overlap between IM Q bank and Q book ..ok... are the Qs in total CK Q bank and the only IM Q bank the same... I will read IM and then will have to give a break ...so Im planning to buy IM Q bank for now... are the Qs same as in total Q bank ? happy studyings all

Dr. Fischer... You are the man!!! I did Kaplan course for CK and your lectures were awsome... I matched last year and as I like to think, it was due to a co-joint effort, including the people who helped me to understand how simple medical concepts can be depending on the perspective... Thanks to you I finally learned how to differentiate leukemias, and I still laugh at the explanation why B Blockers prolong survival in Hear failure. Thank you sir, and best of lucks! Isther

Hi Doctor I've read your ethics book and its really helpful, thanks. I want to ask about a question which wasnt mentioned directly. if a 60-70 alzheimer pt presents with an end stage renal failure, how do we treat him? and what if he was 50? is it the same? Thanks in advance

Hello Children! 1. the Hep C antibody neg, PCR positive person. Repeat the PCR. If it is still positive it may be EARLY disease prior to the antibody being formed. 2. the IM question bank is good for step 2 ck. it is to help you EXCELL!!! and is harder. I did not write those questions, but I DO review them for accuracy. 3. the Alzheimer's with renal failure person: You must dialyze UNLESS the patient previosly stated they did NOT want dialysis when competent OR unless the proxy refuses to have it done. Good luck all! Hi dr , conrad thanks for your time and effort we really appreciate your help i have some questions regrading arythmias hope u help me with them? 1: why there is irregularity in pulse in AF but not in ATRIAL FLUTTER? 2:What is the diastolic current of NA ? is it related to the current injury related to ischemia? 3: why ischemia cuses that current injury? thanks

Dr. Fisher My question is not related to step exam but needed ur expertise on an issue of pulmonology info. How is the MicroRint Device different or similar to using a portable Forced Spirometry Devices which measures FVC, FEV1, FEV2, FER and so on Can the MicroRint Device be used for the same purpose ofmeasuring the same Lung Volumes as Forced Spirometry Devices do?? Are there different in sensitivity, specificity and reliability between the 2. If not know could you direct me to some site where I could acess this info. I Thanks in Advance

Dear Dr. Conrad Fischer, I had a question on whether it is fine to take step 2 ck before step 1. I'm studying abroad and it isn't required for us to take step 1 before step 2 ck. I'm in my final year of medical school and we're taking internal medicine psychiatry, and surgery, and i just completed gynecology/obstetrics and well as pediatrics last year. Do you think that if I use the kaplan lecture notes in conjunction with the video lectures, this would be sufficient to do well on the exam. Of course using USMLE world and Kaplan Qbank as my questions source. The truth is i get mixed answers when asking different people, some say do step 1 first, while others say do step 2 ck since its more relevant to the subjects your taking right now. From what i understand from the kaplan step 2 ck videos is that when something is required from step 1 for example in internal medicine in the rheumatology section, it is explained, such as the cox/lipo-oxygenase pathways. I really would like to know what your opinion on this matter is, that if someone wanted to take step 2 ck before step 1, with the above materials i posted would it be possible to get a good score, thanks a lot. I greatly appreciate it.

Dr. Fischer, first of all thank you so much for such great teaching. I have to take my Step III in 3 months. I messed up my Step II-CK and need to make up for it in Step III. Is it possible for a person to have vibrio parahemolyticus infection and having bloody diarrhea, with leukocytes in stool...isn't as Dr.Conrad Fischer always say not the risk factors but individual symptom? I guess I am not allowed to post the whole question as this is from one of the qbank

Hi Dr. Fischer! I am planning to get into IM residency.. i have been an ER nurse here in the US for 4 years now.. do you consider that as USCE that's required by most programs? do i have to get observership/ externship to have a better chance for the match? thanks!
