psychotic, indigent man w/ a history of multisubstance abuse has been involuntarily hospitalized for 1 week. Because of persisten diarrhea, stools are sent for ova and parasites, revealingg numerous granular, spherical, thin-walles cysts measuring 10-20 mm in diameter. Trichrome stains show up to four nuclei in most of the cysts. These findings are consistent w/ which of the following organisms?
A. Cryptosporidium parvum
B. Dientamoeba fragilis
C. Entamoeba histolytica
D. Giardia lambia
E. Isospora belli

Do not think A. C. parvum is much smaller than 10-20 microns (NOT mm!!!). Measurements are everything with parasites. You MUST measure any potential parasite before even considering identification. C. parvum is too small, and the other parasites mentioned have <4 nuclei. Nuclei are also important in parasites. Another clue - C. parvum does not show up on trichrome staining. You need either a carbol fuchsin acid fast stain or fluorescent stain to see them. So...answer is E. histolytica.

This site gives measurements.....

Apparently you guys are not getting the message. I. belli has thick walls and 2 nuclei.

Key to parasite identification: MEASUREMENTS, number of nuclei, cyst or trophozoite form, patient history.

You cannot make a parasite fit the wrong measurements. You cannot change the number of nuclei. Read the question thoroughly.

Entamoeba page - compares E. histolytica to E. coli - can be difficult for techs to differentiate between the two sometimes. This always shows up on our CAP unknowns.
