thanks zianab and srf but i heard from somebody that if before any program you choose transitional year residancy it would be a +point to go for other residencies..and also all the people can go for it even imgs...

But getting into a transitional program is tough for immigrant ,this is the problem

my Img friend got transitional some years ago and after that went to radiology, scores average to high but not very high.

The issue with the Transitional Year (TY) is this: it will give that person the PGY-1 experience generally required for specialties like Neurology, Radiology, Anesthesiology, Ophtalmology etc. So when you apply you typically say what specialty you have in mind. If you are an AMG you know if your scores are high enough to aim at one of those specialties, if you are an IMG you know that your chances to those kind of programs are small, so why even bother. That is why most Transitional Year programs do not even see too many IMGs applying and that is (I believe) the explanation why the score averages are pretty high for TY (see NRMP report). There is also another reality. Most (but not all) TY programs are based in hospitals that have all the range of those specialties that require PGY-1 and I have seen for instance, a Neurology program in which the applicants are "encouraged" to apply for TY to the same hospital. What does this tell you? That they have unwritten agreements to admit people who later will be admited to those specialties. This is what makes TY so tricky!

the real hitch for an IMG as far as TY prog is concerned is the visa, there is hardly any TY prog which would sponsor a visa for just 1 year, otherwise it is no big deal as far as the scores are concerned, there are plenty of 99x2 around

Right, I overlooked this aspect, as I am awaiting for my GC. Still, at least one TY program I know is willing to give J1.
