anyone tell me detail of mechanism of hypocalcemia in kindey end stage?

There are two mechanisms of hypocalcemia in the patient with chronic renal disease. 1. Chronic kidney disease patients have an inability to excrete phosphorus. Phosphorus forms complexes with calcium and causes a lowering of the level. 2. 1-alpha-hydroxylase in the kidney converts 25-hydroxy vitamin D into the active form, 1,25-hydroxy vitamin D. Vitamin D is of course important for absorption of calcium. So low levels of vitamin D cause decreased absorption of calcium.
