a 28 y/o at 28 wk gesta reports excessive fatigability and dyspnea. her blood pressure is 118/74 mm pulse is 110/min and regular, and lungs are clear to auscultation. the cardiac apex is not palpable. S1 is loud, and there is a sharp sound after S2. Alow frequency diastolic murmur is heard at the apex that increases in intensity before S1. Looking at the left heart: S1 occurs (as mitral valve closes), then ventricular systole (ejecting blood into the aorta, and while this is happening, the left atrium is filling up from the lungs), then S2 occurs (as aortic valve closes and left ventricle goes into diastole). Then the opening snap soon after S2 occurs (as the stenosed mitral valve snaps open, when left atrial pressure exceeds left ventricular pressure). The left ventricle fills up as blood rushes into it through the stenosed mitral valve causing a rumbling diastolic noise.
