I had a doubt withh ccs and would be grateful if u could help me. 1. to discontinue any treatment what should u do...is there any commmand for that or u just go and cancel the order from treatment list....such as a patient on antibiotics for giardia has become alright and u come to the 5 min screen,do u then cancel out the antibiotics from the list or let it be there.? 2. Also what shld be done when the 5 min screen appears,when they ask u to review orders.it would be great if u could tell me for the case of tension pneumothorax and giardiasis as i can atleast practice that....in that particular case 5 min screen comes very fast once u have done the right thngs 3. pt has stabilised and is better....do u cancel out all the orders of actiuive management such as oxygen,iv fluidsetc....plz advise any other tips would be useful thanks

Dear gv5 1. To discontinue any treatment, just click on that order and it'l ask if you want to cancel--say yes. For giardiasis case, if he finished the course of metronidazole, you can stop that- but i guess the 5 min screen will come by then. but, make sure u say -counsel-patient/mediaction compliance/sideeffects/ no alcohol and stuff..... If a case is accepted for surgery, u'll let the previous ordrs stay in there, unlike in a giardiasis ccs where you may want to stop metro, if you go that far as 7 days of metronidazole and you can just do routine counselling.. Hope i am clear here. 2. In real exam, the 5 min screen won't come in so fast, (it won't come in say 4-6 min) bcoz in the practice cd , we usually don't read the entire history, physical and stuff. But in real exam , u'll read them and will be noting down all points relevant to your case, say for eg, in ABDOMINAL aortic aneuysm rupture - they'll say the pt is long term HTN pt & non-compliant with med's. So, u need to note down- non-compliance--as in counseling in last 5 min you want to counsel--medication compliance. in last 5 min :i am pasting the mnemonic i used and i already posted previously in some other forum "rated sex misc (miscellaneous)---choose only relevant for each ccs. everything is not needed in every ccs. reassurance no alcohol no tobacco (smoking) pt education/counselling diet, regular/npo/high protein/low salt/ etc.... seat belt use exercise, regular x--safe sex m--medication counselling/compliance/side-effects i-no illegal drug use s--suicide contract/ suicide precautions/ safety plan c---contraception counselling" 3. pt is better - if u mean completely better-u can stop. but, if u mean stabilized vitals (by ur pt is better)-in that case-- iv fluids will go. above all, imagine as if you are actively treating a patient that moment--this will avoid a lot of mistakes GL
