As a 4-year-old child, this patient had recurrent sino- puhnonary infections with Staphylococcus aureus and Streptococcus pneumoniae; he also developed an arthritis that cleared with immunoglobulin therapy. The germinal centers of his lymph nodes are rudimentary, and he has developed SLE. Which type of cell failed to differenti-ate?


(A) Follicular dendritic cell
(B) Neutrophil
(C) Pre-B cell
(D) Pro-T cell
(E) Stem cell
(F) Type 1 helper T cells (TH1)

i think C b/c dendritic cell will present antigen to helper CD4 T cell which then produce cytokin to stimulate differentiation of preB cell into antibody producing mature B cell or plasma cell.

the answer is c, but doesnt the clinical presntation point towards a common variable immune deficiencey rather than agammaglobulinemia???

i am wrong . actually cytokins stimulate mature B cell to divide and differentiate into antibody producing plasma cell that take place in secondary lymphoid organ as lypmphnode.

ans is (C). Brutons hypogamaglobulinemia.

not common variable because
common variable occurs in late teens to early 20s.germinal center of lymphnode is hyperplastic vs rudimentary in Brutons.
also in Brutons there is association of autoimmune diseaselike SLE, dermatomyositis.Arthriris is belived to be infectious in origin like mycoplasma infection.

but what about arthritis which clears with immunoglbulins?

sorry for late reply.
according to robins path--35% children with brutons develop arthritis that clears with restorative immunoglobulin therapy. this arthritis believed to be infectious in origin.nod
