why not H ???? gastric lavage for AFB ????

PPD is not a diagnostic test for active TB. A positive test only shows latent infection, not active disease. Active TB infection is based on 3 factors: clinical manifestation, CXR, and sputum. In this case, he's clinically asymptomatic (no weight loss, cough, night sweat, hemoptysis, etc), lungs are clear. Next step should be CXR. Sputum for ABF is after high suspicion of infection from history and CXR.
